File of Life Program Sign-up...

Seniors in Geneva Township can sign up to participate in the File of Life program launched by the Geneva Township Senior Services Committee last October. Preprinted forms are provided to record vital medical and health information that will greatly assist emergency medical personnel who are administering care to seniors in their homes. Current prescriptions, doctor’s name and phone number and allergies are examples of information included on the File of Life form. Participants will also receive a file that will be used to store the form in a magnetic case on the resident’s refrigerator which identifies the resident as a File of Life program participant. Participation in this program is free to all Geneva Township seniors 55 years of age or older and program materials can be obtained by calling Geneva Township Senior Services at 630-232-3602.
